Providence Health & Services Patient Navigator in Kodiak, Alaska
Providence is calling a Patient Navigator (Social Services, Day Shift, 1.0 FTE) to Providence Kodiak Island Medical Center in Kodiak, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Greets patients, residents, familes and vistors. Answers telephones and refers calls to appropriate persons. Maintains a daily log of all contact with patients, residents, clients, family members and agencies. Serves as a liaison to patients and their families who are sent outside of Kodiak for medical attention. Assists patients with travel arrangements as needed and maintains knowledge of travel assistance programs and their application process.
Communicates in a compassionate and professional manner to patients, residents, families and visitors. Demonstrates professional interpersonal skills and cooperates with other departments and members of the care delivery team to meet the needs of patients, residents and their families. Demonstrates commitment to teamwork and interacts harmoniously with others. Works collaboratively to ensure team success.
Utilizes available resources. Recognizes errors/problems and takes corrective action. Demonstrates effective priority setting and organization skills to promote department flow and cost efficiency. Ensures smooth, efficient clinic services and maximizes comfort and confidentiality for the patient. Anticipates and prioritizes workflow and adjusts work assignments to prevent delays and meet work flow demands. Assists with problems resolutions.
Updates and maintains available resources files. Maintains knowledge of current residential treatment facilities for substance abuse and mental health and their application process; local resources for food, shelter, low cost housing and rental assistance; and PKIMC’s Financial Assistance, Medicaid, Medicare and Social Security Disability insurances to assist patients/residents in need.
Upon referral from Social Work Services Manager, meets with residents, patients and families to provide assistance with identifying resources needed.
Maintains knowledge of CPSI, Chart Link and the Social Work Services department computer files.
Demonstrates knowledge and support of quality improvement goals. Participates in department projects or committees as assigned.
Recognizes the role that the cultural diversity of patients and their families, employees, medical staff, volunteers and community members play in achieving productive and positive relationships.
Completes Competency Plan for assigned job and department.
Required qualifications for this position include:
High school diploma or equivalent
Current Alaska driver’s license with clean driving record and proof of current vehicle insurance
2 years of experience in customer service
Preferred qualifications for this position include:
- Associates degree in Human Services or related field
About Kodiak Island Medical Center
Providence Kodiak Island Medical Center (PKIMC) provides comprehensive health care to residents and visitors of Kodiak Island. This critical access hospital features 25 acute care beds, including four birthing suites, one OB triage room, and two ICU beds. In addition, the Elder House, PKIMC’s long term care facility, has 22 beds.
PKIMC provides an extensive array of inpatient and outpatient services, including emergency department, surgery, laboratory services, maternity, general medicine, physical therapy, occupational therapy, respiratory therapy, sleep studies, specialty clinics, diagnostic imaging services and pharmacy and is staffed by a mix of primary care physicians, surgeons and specialists who provide family practice, internal medicine, obstetrics, radiology and general practice.
