Providence Health & Services Medical Social Worker in Kodiak, Alaska
Providence is calling a Medical Social Worker (MSW) (ECF Kodiak, Day Shift, 0.5 FTE) to Providence Kodiak Medical Center in Kodiak, Alaska. In this position you will:
ENSURING QUALITY OF CARE: The Medical Social Worker must possess current knowledge of the bio psychosocial aspects of illness, disability and death and be able to apply this knowledge in a professional and understanding manner in direct social work services to patients and their families and in collaboration with all health care professional staff. Has a working knowledge of departmental policies, facility policies and procedures, professional standards, and applicable local, state, and federal laws.
COLLABORATION: As a member of the Care Center multidisciplinary team, provides input of residents’ social and emotional identifiable needs at Care Conferences and weekly rounds. Is able to facilitate ongoing resident council meetings with residents to ensure their rights are understood (implemented). Maintains contact with residents’ representatives (guardian, Power of Attorney, family member) to report on changes in health, current goals, discharge planning, clothing needs and room transfers. Fills in as needed for the Medical Social Work Manager at acute dischargeplanning meetings and swing bed resident care conferences and provides input as to patients’ financial concerns, discharge needs and plans.
FINANCIAL RESPONSIBILITY: Monitors residents’ funds in the resident trust accounts to ensure residents’ Medicaid eligibility is maintained where applicable. Completes annual Medicaid Reviews.
RESIDENT/PATIENT CARE: Assists potential applicants in qualifying for Care Center Admission. Provides support and information to families at the time of resident’s death. Within a strict timeframe completes Social Work MDS sections and quarterly summaries for residents, as well as, a psycho-social assessment and care plan at time of admission and thereafter annually. Upon request, completes a Social Work Consult for acute patient(s).
INFORMATION AND PRACTICAL ASSISTANCE: Receives inquiries and referrals for social services. Screens and secures free or discounted travel for clients who qualify and have off-island medical appointments. Composes letters for clients seeking financial assistance for their medical bills. As time allows assists clients in applying for Social Security Disability and Medicaid Insurance.
CUSTOMER SERVICE: Embraces the Providence Service Standards in all aspects of position. Presents and conducts self in a professional manner. Provides support to coworkers and customers while demonstrating service excellence by building positive interpersonal relations to ensure productive and positive relationships are maximized. Develops a deep understanding of customer’s perspectives and needs and works to meet their expectations. Builds a culture of true caring by behavior and interactions with others.
COMMUNICATION/TEAMWORK: Communicates in a compassionate and professional manner. Demonstrates professional interpersonal and verbal skills when communicating with other department and of the care delivery team to provide optimal continuity of care to patients and their families. Is able to communicate effectively with a wide variety of people from differing literacy levels, languages and cultures. Demonstrates commitment to teamwork and interacts harmoniously with others to ensure team success. Supports new orientees in learning department routines.
Required qualifications for this position include:
Bachelor's degree in Social Work.
Experience with patient and family assessment with underserved populations to include the chronically ill.
Current Social Worker license with the State of Alaska required.
If employee has worked under the Social Worker title for Providence since 2001, licensure is not required.
Must maintain current licensure for appropriate level of education.
Preferred qualifications for this position include:
Master's degree in Social Work.
2 years in Social Worker experience in a hospital setting experience.
