Providence Health & Services Medical Social Worker in Kodiak, Alaska
Providence is calling a Medical Social Worker (MSW) (ECF Kodiak, Day Shift, 1.0 FTE) to Providence Kodiak Medical Center in Kodiak, Alaska. In this position you will:
Possess current knowledge of the bio psychosocial aspects of illness, disability and death and be able to apply this knowledge in a professional and understanding manner in direct social work services to patients and their families and in collaboration with all health care professional staff. Has a working knowledge of departmental policies, facility policies and procedures, professional standards, and applicable local, state, and federal laws
As a member of the Care Center multidisciplinary team, provide input of residents’ social and emotional identifiable needs at Care Conferences and weekly rounds. Is able to facilitate ongoing resident council meetings with residents to ensure their rights are understood (implemented). Maintain contact with residents’ representatives (guardian, Power of Attorney, family member) to report on changes in health, current goals, discharge planning, clothing needs and room transfers. Fill in as needed for the Medical Social Work Manager at acute discharge planning meetings and swing bed resident care conferences and provide input as to patients’ financial concerns, discharge needs and plans
Monitor residents’ funds in the resident trust accounts to ensure residents’ Medicaid eligibility is maintained where applicable. Complete annual Medicaid Reviews
Assist potential applicants in qualifying for Care Center Admission. Provide support and information to families at the time of resident’s death. Within a strict timeframe completes Social Work MDS sections and quarterly summaries for residents, as well as, a psycho-social assessment and care plan at time of admission and thereafter annually. Upon request, complete a Social Work Consult for acute patient(s)
Receive inquiries and referrals for social services. Screen and secure free or discounted travel for clients who qualify and have off-island medical appointments. Compose letters for clients seeking financial assistance for their medical bills. As time allows assist clients in applying for Social Security Disability and Medicaid Insurance

Ensure the Medical Social Work Manager is well informed about social work activities and any problems concerning patient or resident care
Demonstrate effective priority setting and organization skills to promote departmental workflow and cost efficiency. Ensure smooth, efficient services and maximizes comfort and confidentiality for the patient. Anticipate potential obstacles and adjusts work assignments to prevent unnecessary delays. Recognize errors/problems and takes corrective action when necessary. Utilize available resources to recognize errors/problems and takes corrective action and assist with problem resolution
Support the quality control and quality assurance programs by supplying and/or developing indicators to measure, monitor, and report performance to other team members. Seek input from various customer groups to maintain quality. Report any performance improvement suggestions to the department leader
Ensure an effective purchasing process is maintained by requisitioning necessary products and supplies for personnel, determining necessary quality/quantity requirements, selecting suppliers according to current contracting, arranging for deliveries, following up on orders, dispensing of equipment and other items when necessary. Develops and/or maintains department records keeping system, regarding par levels of department supplies. Submit invoices for approval and processing in a timely manner
Assist with responsibility for obtaining information and expanding job knowledge and competency that reflects current practice. Role models personal development and education. Evaluate own practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations
Assists in training of non-social work employees in areas of expertise, i.e. elder abuse and neglect, end of life decisions, and identifying spiritual needs
Demonstrate knowledge and support of quality improvement goals. Participate in department, facility and organizational projects or committees as assigned
Implement care/services that recognize age/diversity specific needs/issues of customers served
Complete Competency Plan for assigned job and department
Required qualifications for this position include:
Bachelor’s Degree in Social Work
Current Social Worker license with the State of Alaska
If employee has worked under the Social Worker title for Providence since 2001, licensure is not required
Must maintain current licensure for appropriate level of education
Experience with patient and family assessment with undeserved populations to include the chronically ill
Preferred qualifications for this position include:
Master’s degree in Social Work (MSW)
2 or more years Social Worker experience in a hospital setting experience

This critical access hospital features 25 acute care beds, including four birthing suites, one OB triage room, and two ICU beds.
